EPIC Rehabilitation and Nursing at White Plains is a NEW Skilled
Nursing & Rehabilitation center located in the heart of White
Plains. This SNF is part of our " Epic Healthcare Management" group
of facilities.The role of the Licensed Practical Nurse is to
provide day to day support to the Unit Manager/Charge
Nurse/Supervisor, respond appropriately to resident/family needs
within the scope of practice and organizes and oversees the
assigned unit activities and staff assignments ESSENTIAL JOB
RESPONSIBILITIES: During the course of your shift you may be
required to perform some or all of the following:ESSENTIAL JOB
RESPONSIBILITIES: During your shift you may be required to perform
some or all the following:1. Utilize the nursing process in the
care of designated Residents.Assure Assessment/Data Collection
Documents clinical data on all assigned Residents per
regulations.Report findings of ongoing changes /concerns to the
Charge Nurse, Nursing Supervisor or Nurse Manager.Demonstrate
knowledge of basic lab/test collections.Planning: Collaborate with
RN/Interdisciplinary Team to revise/update the Nursing Care
Plan.Implementation: Deliver direct Resident care consistent with
the policies and procedures of the Facility. Demonstrate skills
necessary to deliver direct resident care. Administer medications
and treatments in accordance with policy and procedure. Deliver
care incompliance with Resident's Rights and federal/state
regulations.2. Respond appropriately to Resident/Family needs
within the scope of practice.Identify Resident/Family responses and
notifies designate RN in timely manner. If appropriate, consult
with other members of the health care team.Monitor and document
vital signs, treatments administered and Residents'
responses.Demonstrate knowledge of equipment and procedures.LPN may
not flush Picc lines, subclavian lines, implant ports or change
dressings or draw blood from any of these lines.3. Ensure the
safety of all Residents in accordance with Facility/Nursing policy
and procedure.Prepare Resident assignments for Certified Nursing
Assistants, considering the category of needs of the Resident and
competency of personnel.Maintain equipment on unit and reports
malfunctioning promptly.Adhere to fire plan, disaster plan and
other safety plans established.Address and communicate lab results,
test results to RN supervisor, unit manager and MD.Communicate
Residents' incidents and concerns to RN supervisor unit manager and
MD.4. Demonstrate effective interpersonal skills necessary to
ensure quality patient care.Interact with Residents, Families, and
colleagues in a courteous and professional manner.Always maintain
Resident privacy and confidentiality.Demonstrate an awareness of
Resident's rights and reports any concerns.5. Organize and
supervise assigned unit activities and staff assignments.Provide a
Nursing Assistant assignment for each CNA on his/her team.
Communicate to CNA pertinent resident care needs.Monitor and
evaluate the flow of activity and delivery of patient care provided
by the CNA's (turning and positioning, mealtimes, bathing,
etc.).Assure CNA documentation records (ADL's BM, Meal intake, etc)
are completed. Communicate to Charge Nurse/Unit Manager and other
member of the care team any pertinent resident findings.Assist in
providing educational needs of staff. Communicate education needs
for follow-up to Charge Nurse/Unit Manager/In-Service Director.6.
Recognize job relationships/demonstrate a willingness to function
within Nursing Home Structure.Utilize appropriate lines of
authority and methods of communication in problem solving.Refer
problems, which are beyond the scope of LPN authority, ability, and
legal practice to the Registered Professional
Nurse/Administration.7. Demonstrate accountability in professional
practice developmentAttend scheduled facility in-service
programs.Meet annual mandatory in-service requirements, including
but not limited to CPR, IV course/ recertification, Infection
Control etc.8. Demonstrate accountability in practice to Residents,
family, colleagues, and the facility.Adhere to established
professional Dress Code.Is punctual and/or provides proper
notification for lateness or absenteeism.Utilize benefit time
appropriately.Utilize facility resources in an efficient,
cost-efficient manner.Demonstrate an understanding of department
priorities related to patient needs.When necessary, work a flexible
schedule to provide continuity of care for all patients.Assist
other staff members in completion of their duties.Orient new
Nursing personnel as assigned.9. Ensure all HIPAA requirements are
adhered to.10. Follow all facility policies and procedures.11.
Perform other related duties as required and assigned.Requirements:
Education:Must be a graduate of an approved school of
Nursing.Licensed and currently registered in good standing in New
York State as Licensed Practical Nurse.Work Experience:Previous
experience with geriatric patients is desirable.Job
Knowledge/Requirements:Must have a sincere desire to work with the
elderly, chronically ill, and disabled.Pharmacological background
in theory and supervised clinical experience with
medications.Thorough knowledge of principles and methods involved
in handling of sterile/clean materials and daily hygienic care of
residents. Preventing spread of disease by proper disposal of
infected material and cleaning of equipment.First aid assistance in
emergencies, must utilize good judgment in emergency
situations.Must have a thorough knowledge of medical, social, and
psychological needs of patients.Must possess a spirit of
cooperation, caring and enthusiasm in order to create an atmosphere
conducive to rehabilitation, optimal health and growth.ADA
Requirements:Candidate will have the physical ability to perform
job related duties, which may require lifting, bending, stooping,
stretching, pulling and pushing up to 50lbs or more as needed.EEO
