• Job Details
• Clinic RN / Larchmont Office at White Plains Hospital
• Job requires the ability to perform clinical/technical/service/administrative tasks
• Responsibilities include assessing patient physical, psychosocial and spiritual health status to determine healthcare and educational needs
• Providing and/or delegating patient care according to care goals and setting priorities based on patient needs
• Assists with triage of sick and walk-in patients
• Establishes therapeutic and supportive relationships with patients and family
• Acts as a resource person and coordinates client care, including referral back to primary provider
• Involves in telephone triage, handling prescription requests, and communicating test results
• Performs therapeutic treatments and assists with procedures
• Administers medications and coordinates all patient care activities
• Coordination of in-patient admissions and ER referrals
• Advocacy for patient needs and ensures patient safety
• Participation in performance improvement activities and educational presentations
• Works as a member of the patient care team and engages in daily huddles and practice team meetings
• Attends in-service and continuing education programs
• Displays professionalism and courteous demeanor with patients and team
• Informs and educates patients/families for participatory care
• Evaluates intervention effectiveness and patient responses
• Collaborates with team for patient care adjustments
• Communicates patient status changes to care team and health care providers

  • BSN required
  • Minimum $1.25 an hour for parking

    • Variable: Mon-Friday 8am-4pm, 9am-5pm, 10am-6pm
