Explore our open positions and apply to become a part of the Summit Health and CityMD family.Job DescriptionThe Advanced Nurse Practitioner will assist in the daily medical care, admissions, and discharges of patients on the Summit Health hospitalist service. The APN will work in close supervision with the physician staff of the Summit Health hospitalist department. The APN will provide primary assistance to the post-call hospitalist physician, or the on-call hospitalist physician as determined by the Hospitalist Lead/Service Line Chief. The APN will provide support to the physician staff to ensure achieving value- based quality performance metrics such as, quality evidenced based care, right post acute setting, patient safety, decreased length of stay, reduce readmission rates, etc.Essential Job functions:

• Identifying chief complaints/issues and directing medical care for patients in an inpatient setting
• Providing detailed, focused medical assessments and management of patients and transition them back to ambulatory settings from the hospital
• Collaborating with APN Collaborative Practice and Joint Protocol Agreement or PA Supervisory Agreement to review acute medical illness, assess expected recovery, evolve care plan, and educate patients and families
• Collaborating with RN Care Manager, Social Worker, Clinical Pharmacist, Post-Acute APP, PAC RN as needed to meet patient care and social needsCompensation Range: $118,000-$135,000The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience, individual performance, quality measures and location of position.#joinsummithealthTotal Rewards at Summit Health CityMDOur team members are essential to our mission to reshape healthcare through the power of connection.
