Job Description:

The Clinical Pharmacist is responsible for the effective management of rational antibiotic therapy within the facility or the management of critical care or high-risk patients, as outlined by the specific area of responsibility. The role involves collaborating and consulting with other healthcare professionals regarding the therapeutic use of antibiotics or other medications for patients within the scope of responsibility. Requirements: Degree from an accredited school of Pharmacy with a PharmD degree. Current New York State Registration. Responsibilities: Understand and adhere to Performance Standards, Policies, and Behaviors. Enhance the monitoring and management of the clinical pharmacy antibiotic service or critical care for high-risk groups, implementing interventions to improve the quality and cost-effectiveness of patient care. Promote the clinical pharmacy development of pharmacy staff through clinical leadership and education. Work collaboratively with others to achieve organizational objectives, fostering interpersonal and team relationships to establish common ground, build shared goals, and encourage new ideas. Generate creative approaches and new ideas to enhance performance, service, and quality. This includes identifying and evaluating options and interventions, involving relevant individuals in the decision-making process, and making informed decisions based on comprehensive information. Take personal responsibility for all work activities and actions, ensuring follow-through on commitments and implementation of agreed-upon decisions. Demonstrate understanding and sensitivity towards others by treating people with courtesy and respect. Establish and maintain effective relations through friendly greetings and a positive outlook. Display a high degree of honesty, loyalty, and integrity. Foster collaboration and trust among colleagues and departments, contributing to a positive work environment aligned with the Mission and values of the Hospital. Maintain open dialogue by actively listening to others and responding positively to requests, questions, and suggestions. Share information in a timely manner and keep others adequately informed. Practice cost containment measures. Adhere to all hospital policies and procedures, including the dress code, behavioral standards, conduct, and customer service, meeting all mandatory in-services and standards for attendance. Perform all other related duties as assigned.
