Family Practice-Without OB opening in White Plains, New York. A healthcare facility located in Westchester County is actively seeking a patient-oriented and driven Primary Care Nurse Practitioner (NP) to join their growing staff. In this role, the Primary Care Nurse Practitioner (NP) will be responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients. Responsibilities The Primary Care Nurse Practitioner (NP):

• Completes a comprehensive initial assessment
• Performs all activities for multidisciplinary care coordination at the intra-hospital level of care and inter-hospital level of care
• Performs all transitional planning activities
• Communicates and collaborates all relevant patient information to appropriate health team members
• Ensures department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized
• Documents per department requirements
• Participates in Performance Improvement activities as needed
• Performs all UR activities per department policy Qualifications
  • MSN
  • Active New York State license to practice as a Registered Professional Nurse
  • Current New York State license as a Nurse Practitioner
  • National Certification as a Nurse Practitioner
  • Current collaborative practice agreement
  • BCLS certification Desired skills
    • 2+ years' experience in an acute care setting
    • ACLS certification
