Job Description:

White Plains Hospital, a 292 bed, leading award-winning Magnet hospital, located 25 miles north of Manhattan, in White Plains, Westchester NY, is looking for a NY licensed Palliative care Nurse Practitioner (NP) to join our Palliative Medicine Department.
Schedule: Full time Day, -M/F 8am to 4pm
This position is - part of the interdisciplinary team responsible for inpatient consultations. -The department is also responsible for a growing ambulatory palliative practice at our Comprehensive Cancer Center. -
Primary responsibilities for this position include providing palliative medicine consultation to a diverse population inpatient and outpatient settings and participation in program development to promote integration of palliative services and principles throughout the organization.

      PAIN & PALLIATIVE CARE NP JOB RESPONSIBILITIES

      • The Palliative Medicine Nurse Practitioner provides clinical management & support, education and development primarily in the hospital setting. The NP may function between the in patient service and the outpatient cancer center program at the request /direction of the Palliative Medicine Director. -
      • Provide clinical, educational and emotional support as appropriate to patients and their families living with serious illness.
      • Facilitate clarification of patient and family goals of care including complex medical decision-making and clarification of advance directives.
      • Provide pain and symptom management.
      • Facilitate access to appropriate support models of care while in hospital and upon discharge.
      • Provide second opinion regarding appropriateness for hospice care as an acute in patient and at time of discharge as well as information regarding hospice and other palliative resources within WPH and in the community.
      • Act as a resource person, consultant, educator and facilitator for the entire Health Care Team specific to palliative interventions and principles in serious illness.
      • Perform physical examinations and preventive health measures within prescribed guidelines
      • Participate in after hours on call responsibilities by telephone as needed and per schedule.
      • Prescribe and adjust medical management regimen specific to pain and symptom management and within the scope of practice for an NP and certified palliative care APRN.
      • Interprets findings and plans of care according to collaborative practice agreements and protocols.
      • Initiate referrals to specialty services and foster continuity of care consistent with consultation focus and in agreement with referring provider. - Additionally, serve as a liaison / content expert to assist in transitions of care for patient's not seen by Pall Med Service as a support to WPH quality care initiatives.
      • Provides patient and family education and counseling both individually and/or as a family regarding the palliative care philosophy and treatment options.
      • Participates in team meetings and conferences to enhance a multidisciplinary approach to patient care at the request of Director of Palliative Medicine Service.
      • Provides educational programs and resource materials as required/requested on issues related to serious illness and palliative interventions at the request of Director of Palliative Medicine Service including but not limited to:
      • Nursing Staff
      • General WPH Staff
      • Interdepartmental programs
      • Medical Staff
      • Patients, families and community
      • Collaborate with all members of the Health Care Team (Nursing, Medical Staff, Pharmacy, Nutrition, Social Services, Discharge Planning, Home Care, Purchasing, etc.) to assure quality care for patients of all ages in need of palliative interventions has the information and access to consultation when appropriate.
      • Contribute to the development of standards of practice for palliative interventions at the request of Director of Palliative Medicine Service.
      • Performs other duties as assigned at the request of Director of Palliative Medicine Service.

        REQUIREMENTS

        • Master's Degree in Nursing from an accredited school.
        • Current NYS license to practice as a Registered Professional Nurse (RN).
        • Current NYS certification as a Nurse Practitioner (Adult/Geriatric or Family).
        • Minimum two years' experience in an acute care setting.
        • Minimum of one year Palliative Care experience.
        • In addition, Critical Care experience is strongly preferred.
