Job Description:

Purpose of Position:In conjunction with the Director of Patient Services, is responsible for Outcome Based Quality Improvement/Quality Monitoring(OBQI/OBQM) and Staff Development activities within the clinicaldepartmentsResponsibilities:1.Develops, implements and evaluates ongoing quality improvement activities that support the Agency's objectives in delivering high quality, safe, cost effective home health services. These activities are to reflect the home health agency's philosophy, standards of practice and administrative goals.2.Develops, implements and evaluates the Staff Development program for the clinical departments based on OBQI/OBQM activities and staff needs.3.Oversees OBQI activities including analysis of global outcomes and intermittent outcomes.4.Oversees OBQM activities including the analysis of quarterly potentially avoidable events.5.Coordinates Quality Improvement activities with the Director of Patient Services to maximize utilization of Information System and personnel resources, and maintains a high level of communication between all departments.6.Identifies opportunities to improve care through the process of care investigation utilizing OBQI/OBQM reports.7.Develops and implements a plan of action related to the process of care investigation.8.Monitors plan of action on an ongoing basis.9.Identifies customer needs through patient, physician and referral source satisfaction surveys.10.Oversees maintenance of regulatory standards including timely completion of all documents, adherence to Medicare Conditions of Participation and CHAP standards.11.Investigates and trends complaints and incidents.12.Oversees of QI Department, including Nurse Auditors and Data Entry Staff.13.Acts as liaison with the Billing Department to ensure timely flow and processing of all documents and billing issues to ensure financial stability.14.Serves as Chairperson of the Quality Improvement Committee, reporting to the Professional Advisory Committee at their quarterly meetings.15.Encourages participation from all levels of staff in the Quality Improvement process.16.Assists the Director of Patient Services in promoting and maintaining an agency environment that is in compliance with Federal and Staff regulations, in addition to maintaining CHAP standards of excellence.17.Participates in professional development activities.18.Performs other duties as assigned.Supervisory Responsibilities: Directly supervises all employees in the Quality Improvement Department in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees, planning, assigning and directing work; appraising performance: rewarding and disciplining employees; addressing complaints and resolving problems.Qualifications:1.License and current registration to practice as a Registered Professional Nurse in the state of New York.2.Bachelor's Degree in Nursing, Masters Degree, preferred from an accredited college or university.3.Five (5) years in a CHHA or LTHHCP; at least two (2) years as a Clinical Manager.4.Demonstrated competence in OBQI/OBQM activities. Ability to develop and interpret management reports.5.Strong interpersonal and written communication skills.6.Ability to be self-directed and work independently.7.Knowledge of Horizons Homecare computer software and the ability to conduct staff training programs preferred.8.Proficient in the use of Microsoft Word, Excel and Power Point.9.Professional liability insurance in the amount specified by the Agency.10.Automobile in good working order and automobile insurance in the amount specified by the Agency.
