Qualified candidates will have recent adult acute care, Hospital, Med/Surg, ER experience and/or Hospice and Home Health RNs will be considered. Nurse Practitioners are encouraged to apply! Openings available for Manhattan, NY Visits or Lower Westchester County, NY visits!RN Home Case Manager - Registered Nurse - Private Care at HomeRN Home Case Manager, Registered Nurse is responsible for starting the patients care, including performing a comprehensive nursing assessment, medication reconciliation, safety assessments and implementing standardized protocols. This high-acuity, private care is provided in the comfort of the patients home.Position is in the field, Monday - Friday 9AM - 5PM, with paid telephonic on-call weekend coverage, on a rotating schedule.ResponsibilitiesParticipates in admissions and performs a timely start of care visits for new admissions to the program.Performs comprehensive patient assessments and provides patient care.Performs medication reconciliation, Trach, IV, colostomy, Assessment and treating of surgical incisionsParticipates in calls with treating clinical teamAdministers prescribed medications (typically intravenously), obtains blood samples, and provides therapeutic interventions.Educates patients on how to manage their illness, prescribed medications, nursing interventions, and care plan.Completes Private visits to frail, adult population in a home setting with support of Clinical TeamQualificationsAssociate of Science in Nursing, BSN PreferredCurrent, unencumbered state license to practice as a Registered Nurse.1 year of adult, acute, inpatient medical, or medical/surgical care experience within the last five years.Current CPR certification.Ability to use Public Transit for Manhattan VisitsExperience providing RN services in the home, community, or clinic, preferredAdult medical critical care, preferredExcellent Salary $120,000-$130,000 & Benefits!Qualified Candidates Please Apply Now for Immediate Consideration!#IND1by Jobble
