Job Description:

A health services provider in New York is actively seeking a licensed Registered Nurse (RN) to work throughout Rockland -and Orange County as their new Case Manager.
-
Responsibilities:

The Case Manager will:

• 
  
• Develop and evaluate the POC
  
• Provide optimum patient care in an efficient and cost-effective manner
  
• Participate in the orientation of newly appointed employees
  
• Assure the delivery of safe, therapeutic patient care in accord with company policies and professional nursing standards -
  
• Assess patient care needs and intervenes to ensure that needs are met in an appropriate and timely manner
  
• Collaborate with clinical and support staffs to effectively coordinate the daily care of the patient: informs physician and other personnel of changes in patients' needs and outcome of interventions
  
• Contribute to patient and family counseling and education
  
• Participate in ongoing interdisciplinary comprehensive assessment
  
• Document assessments, visits, all communications/conferences/IDT meeting, care plans and Discharge Summaries per UHR standards
  
• Perform other duties, as needed
  
  Qualifications:
  
  
  • 
    
  • 2+ years of Medical-Surgical Nursing experience; 1+ year of Bedside Nursing experience
    
  • Graduate of an accredited School of Nursing
    
  • NYS Registered Nurse (RN) license
    
  • Experience caring for terminally ill patients and a basic understanding and commitment to the hospice concept
    
  • Basic knowledge of medications, treatments & therapies for Pain & Symptom Control
    
  • Computer savvy
    
  • Solid assessment, clinical, and documentation skills
    
  • Patient oriented
    
  • Great interpersonal skills
    
  • Excellent communication skills (written and verbal)
    
  • Strong atteniton to detail
    
  • Highly organized
    
    Desired Skills:
    
    
    • 
      
    • Bachelor's Degree in Nursing
      
    • Experience in a Home Care, Hospice and/or Oncology role/setting
