Seeking experienced, full-time or part-time RNs with a minimum of 10 years of clinical experience to join our fast-growing team. Enhance the quality of life for our senior clients and individuals with disabilities through life care management services that address the medical, environmental psycho-social, and financial needs of our clients while respecting and advancing clients' independence and quality of life. Job entails developing and implementing care plans, wellbeing assessments, care coordination, care oversight, healthcare advocacy, interaction with routine and emergency care providers, community resources, and families. This is NOT home health care nor are we providing any hands-on care. Rather, you are here to develop care strategies/plans, implement these plans and provide quality assurance of all facets to optimize our clients' quality of life. Provide onsite (clients' home or within the facility in which they reside) medical, environmental and psycho-social assessments of the assigned client using the IKOR Assessment Forms and Procedures. Document Care Plan recommendations and work with other IKOR employees and family members to execute plan. If requested by client, attend doctor appointments, therapy appointments, facility team meetings, and/or family conferences as dictated by the needs of the case and the parameters set forth in the IKOR Agreement of Service. Attend hospital, rehab and SNF discharge planning meetings with/on behalf of clients. Interface with all other IKOR team members to ensure smooth coverage of client needs while avoiding redundant efforts across the team. Maintain detailed, timely, accurate and case notes in client case note system, and carefully log client-related expenditures to ensure proper reimbursement to IKOR. Attend regular meetings of the entire IKOR staff for in-service, training and/or staff development. Schedule is flexible and generally between the hours of 9-6, Monday through Friday, but some weekend and/or crisis work during off-hours should be expected Very competitive pay structure consistent with experience, with overtime/after-hours pay. Qualities of our RN Advocates: Bachelor's Degree required with advanced degree (e.g. MSN, etc.)/certifications preferred as a differentiator. Prior experience as Care Manager (CCM or similar), geriatric care and/or working with individuals with disabilities strongly preferred Previous work experience to include ER, ICU, CCU, or similar critical care environment Organized, critical thinker with the ability to anticipate potential issues and risks Strong communication skills, attention to detail, and professional personal presentation at all times Valid driver's license and personal transportation required.
