Job Description Weekend / Field RN / Coordinator of Care/n
Salary: $95,000 - $110,000, plus a $15,000 sign-on bonus.
/nLocation: Brooklyn, NY /nFriday, Saturday and Sunday (13 hr
Schedule) /nOrganization: Large, Respected Certified Home Care
Agency (CHHA) /nRole Overview /nThe Field RN/Coordinator of Care is
a vital member of the home care team, managing all aspects of
patient care. This full-time, permanent position follows a Monday
through Friday schedule, offering stability and flexibility.
Responsibilities include conducting home visits, developing and
implementing care plans, coordinating with interdisciplinary teams,
and ensuring accurate and timely documentation. /n /nKey
Responsibilities /n /n/n /n/t - /n/t Perform home care assessments
to determine eligibility and develop care plans. /n/t /n/t - /n/t
Provide skilled nursing care, including wound care, medication
management, and disease education. /n/t /n/t - /n/t Coordinate and
communicate with patients, families, physicians, and
interdisciplinary teams. /n/t /n/t - /n/t Complete documentation in
HCHB clinical software within required timeframes. /n/t /n/t - /n/t
Supervise and orient home health aide personnel as needed. /n/t
/n/t - /n/t Identify needs for additional services (e.g., PT, OT,
MSW, speech therapy) and coordinate accordingly. /n/t /n/t - /n/t
Monitor patient progress, adjust care plans as needed, and
participate in case conferencing. /n/t /n/t - /n/t Ensure
compliance with infection control standards and agency protocols.
/n/t /n /n/n Requirements /n/n /n/t - /n/t Registered Nurse (RN)
licensed in New York State. /n/t /n/t - /n/t Minimum of 6 months
clinical, hospital experience. /n/t /n/t - /n/t Vehicle required
for most territories. /n/t /n/t - /n/t Familiarity with EMR
systems; experience with HCHB is a plus. /n/t /n/t - /n/t Current
